How To Register
In order to register with Middleton Health Centre you need to fill in our registration forms and attend a new patient health check (if you are over 15 years old). The forms can either be obtained from our reception desk or you can download them from this page.
If you download the forms, give us a call to make your new patient check appointment.
During this new patient check we will take some basic measurements such as your height and weight and then take a brief medical history from you. It’s nothing to worry about, we simply want to get to know you and your health a little better.
The health check usually takes around 25 minutes or less to complete and once its done you will be registered with the surgery and be free to make appointments with any of our Doctors and other clinical staff.
For those aged between 40 and 74, we will undertake a fasting blood test to check your cholesterol and test you for diabetes. These checks are part of the standard NHS Health Check offered to those aged between 40-74 every 5 years.
