Our practice is a safe space and if you need to use our building as a place of refuge then you are encouraged to do so.

We know that life can change quite quickly and be complicated to explain over the phone or face to face, and so if you would like to let us know about something that has changed recently in your life, then please use this form to do so.

Your message will be screened by the reception team and your medical notes will be updated, if appropriate, so that the clinical staff are aware of your situation.

You can ask to be contacted by a member of the team if you wish to discuss your circumstances further and get any advice.
